A driving medical report from a GP is a document that assesses a person's fitness to drive based on their medical condition. It is typically requested by a driver's licensing authority or insurance company to determine if a person with a medical condition can safely operate a vehicle.
Please request your medical history from your GP and bring this with you on the day of the appointment.
Appointment cost: £100
A GP at our Lurgan clinic assesses whether a medical condition affects your fitness to drive, then prepares the report the licensing authority or insurer has asked for.
What the appointment includes
The fee is £100. That covers a 20 minute appointment and the written report. People book this for taxi driving, race driving, crane work and other vehicles where a medical is part of the application.
This is separate from an HGV, LGV or D4 driver medical. Those forms are completed on the day of that appointment. If you are not sure which one you need, call us before you book.
What to bring
Ask your own GP for a copy of your medical history and bring it with you. If the examination uses a DLM1 form, bring that too, so the doctor can complete it during the appointment.
The clinic is at 41 High Street, Lurgan, Craigavon BT66 8AH. Appointments run Monday to Friday, 9am to 5pm, and Saturday, 9am to 1pm.
