A sports medical report from a GP is a document that assesses a person's fitness to participate in a particular sport or physical activity based on their medical condition. It is typically requested by sports organisations, coaches, or trainers to ensure that the person is physically capable of participating in the sport without putting themselves at risk of injury.
Please request your medical history from your GP and bring this with you on the day of the appointment.

A GP at our Lurgan clinic assesses whether you are medically fit for a sport or activity, then writes the report the club, coach or organiser has asked for.
What the appointment includes
The fee is £100. That covers a 20 minute appointment and the written report. Clubs and organisers ask for this report for boxing, wrestling, running, swimming, motor sport and other activities.
The report is about fitness to take part. It is not a sports injury clinic. If you are already injured, our sports injury physiotherapy team is the right place to start.
What to bring
Ask your own GP for a copy of your medical history and bring it with you. If the club has given you a form, bring that too, so the doctor can complete the sections they need.
The clinic is at 41 High Street, Lurgan, Craigavon BT66 8AH. Appointments run Monday to Friday, 9am to 5pm, and Saturday, 9am to 1pm.
