Medical reports from a GP provide information about a patient's medical history, current health status, and any diagnoses or treatments they have received. These reports may be requested by other healthcare providers, insurance companies, government agencies, or other third parties for various reasons.
Please request your medical history from your GP and bring this with you on the day of the appointment.
Appointment cost: £100
A GP at our Lurgan clinic prepares the medical report an employer, embassy, insurer, lender or other organisation has asked you to provide.
What the appointment includes
The fee is £100. That covers a 20 minute appointment and the written report. People use this appointment for pre-employment checks, visa and immigration applications, insurance, mortgages, and specialist activities such as diving.
Bring the letter or form that says what the organisation needs. The GP completes the report from your history and the examination. If the form asks for something this appointment does not cover, we will say so before you book.
What to bring
Ask your own GP for a copy of your medical history and bring it with you, together with the form or the request from the organisation.
The clinic is at 41 High Street, Lurgan, Craigavon BT66 8AH. Appointments run Monday to Friday, 9am to 5pm, and Saturday, 9am to 1pm.
